How does Renvela work?

Sevelamer binds to phosphorus in your diet and prevents it from being absorbed into your bloodstream.

Side Effects of Renvela

Nausea, vomiting, upper abdominal pain, constipation, diarrhea, and bloating.

• These side effects may decrease over time. If they persist or worsen, consult your doctor.

Quick Suggestions:

• Avoid or limit high phosphorus addictive foods (processed meats, instant puddings, sauces, spreadable cheeses, and beverage products).
• Avoid intake of alcoholic beverages with Sevelamer 800 as it can make you dehydrated and may increase the side effects.
• Make sure you drink plenty of fluids. This will help you clear out the extra phosphate and will help you overcome some.
• Sevelamer 800 is preferably taken with meals as it can bind to dietary phosphate and prevent its absorption into the bloodstream. Drink it with a full glass of water.
• Do not stop taking the Sevelamer 800 dose until your doctor says so.
• Do not use Sevelamer 800 if you have a history of bowel obstruction.

Indication of Renvela

• Control of hyperphosphatemia in adults receiving hemodialysis or peritoneal dialysis.
• Control of hyperphosphatemia in adults with chronic kidney disease (CKD) not on dialysis, with serum phosphorus ≥1.78 mmol/L.
• Should be part of a comprehensive treatment approach, potentially including calcium supplements and vitamin D3 or analogs to manage the renal bone disease.

Pharmacology

Sevelamer carbonate is a non-absorbed phosphate-binding polymer free of metals and calcium. It binds phosphate in the gastrointestinal tract through ionic and hydrogen bonding, reducing phosphate absorption and lowering serum phosphorus levels.

Dosage & Administration of Renvela

For End-Stage Renal Disease (ESRD) Hemodialysis and Hyperphosphatemia:

Initial Dose:

• Serum PO4 >9 mg/dL [2.91 mmol/L]: 1600 mg orally every 8 hours with meals.
• Serum PO4 7.5-9 mg/dL [2.42-2.91 mmol/L]: 1200-1600 mg orally every 8 hours with meals.
• Serum PO4 5.5-7.5 mg/dL [1.78-2.42 mmol/L]: 800 mg orally every 8 hours with meals.

Maintenance Dose:

• Serum PO4 >5.5 mg/dL [>1.78 mmol/L]: Increase dose by 400-800 mg per meal.
• Serum PO4 3.5-5.5 mg/dL [1.13-1.78 mmol/L]: Maintain current dose.
• Serum PO4 <3.5 mg/dL [1.13 mmol/L]: Decrease dose by 400-800 mg per meal.

Dosing Considerations:

• Titrate the dose by increasing it by 400-800 mg per meal at 2-week intervals.
• Do not exceed a total daily dose of 4 grams.

Switching from Calcium Acetate:

• Substitute 800 mg of Sevelamer 800 for 667 mg of calcium acetate.
• Substitute 1600 mg of Sevelamer 800 for 1334 mg of calcium acetate.
• Substitute 2400 mg of Sevelamer 800 for 2001 mg of calcium acetate.

Pediatric Dose:

• The safety and efficacy of Sevelamer 800 in children have not been established. It is not recommended for use in pediatric patients.

Interaction

Drug-Drug Interactions:

• Ciprofloxacin: Sevelamer 800 may decrease the absorption of ciprofloxacin. It should not be taken simultaneously with this antibiotic.
• Mycophenolate: Sevelamer 800 may affect the absorption of mycophenolate. Monitor blood concentrations if used together.
• Levothyroxine: Sevelamer 800 may reduce the effectiveness of levothyroxine. Monitor thyroid-stimulating hormone (TSH) levels.
• Erdafitinib: Interaction with erdafitinib may occur; consult your doctor.

Drug-Food Interactions:

• High-Phosphate Additives: Avoid high-phosphate additives in your diet as they can interfere with Sevelamer 800’s effectiveness.
• Alcohol: Avoid alcohol consumption as it can increase side effects.
• Multivitamins with Minerals: Sevelamer 800 may decrease the effectiveness of multivitamins with minerals. Take multivitamins at least 4 hours before or after taking Sevelamer 800.

Drug-Disease Interactions:

• Hypophosphatemia: Sevelamer 800 is contraindicated in patients with low phosphate levels.
• Bowel Obstruction and Perforation: Sevelamer 800 should not be used in cases of bowel obstruction or perforation.
• Severe Liver Disease: Use Sevelamer 800 with caution in patients with severe liver disease.

Contraindications

Do not use if allergic to any ingredients in Sevelamer 800.

Pregnancy & Lactation

Pregnancy and Breastfeeding: Use Sevelamer 800 with caution if you are pregnant or planning to become pregnant. It is not recommended for breastfeeding mothers, as it may pass into breast milk.

Precautions & Warnings

• Avoid use if you have low phosphate levels in your blood.
• Not suitable for those with bowel obstruction.
• Contains lactose (milk sugar). If you have lactose intolerance, consult your doctor.
• Inform your doctor if you have swallowing problems, motility issues, frequent vomiting, active inflammatory bowel disease, or have had major stomach or bowel surgery.
• Pregnancy and Breastfeeding: Consult your doctor if you are pregnant or breastfeeding before using Sevelamer 800.
• Do not use Sevelamer 800 if you are allergic to it.
• Avoid alcohol while taking Sevelamer 800 as it may cause unpleasant side effects.
• Sevelamer 800 should not be given to children under six years of age, as its safety and effectiveness in this age group have not been established.
• Regular monitoring of serum phosphate levels, and levels of vitamins D, A, E, K, and folic acid is advised while taking Sevelamer 800.
